PEth is measured from blood after a draw handled like other occupational specimens. It reflects a different biological window than a single breath snapshot. Employers use it when their written plan and clinical partners agree that blood monitoring answers the question—not because breath is inconvenient.
Where employers usually see PEth in the real world
Return-to-duty style monitoring, fitness-for-duty discussions managed with counsel, specialty occupational health packages, or agreements that already include periodic blood work are common contexts. Random floor screening for every employee is rarely the first conversation.
If a third-party administrator routes orders, confirm how PEth appears on the requisition, how results return, and how your medical review officer or reviewing physician expects to handle prescription overlap.
Logistics HR and operations should plan for
Blood draws need private space, chair access, and the same dignity you would offer for any medical service on site. Some employers schedule draws inside occupational health clinics; others coordinate mobile visits when policy and vendor agreements allow.
Chain of custody still matters. Name who signs as employer representative and how you transport or ship specimens under your lab instructions.
What PEth is not
It is not a silent replacement for Department of Transportation breath alcohol rules. Regulated drivers still follow the procedures your DER manages.
It is not a personality test or a stand-in for supervisor coaching. It is one tool in a documented program.
